The template is a Googledocs spreadsheet located here.
You will need to make a copy of the file before you can work on it. Instructions are provided in the file, hover over cells to see notes for further explanation.
Make sure you actually follow these instructions, otherwise, the page you produce may not be approved.
If you have any trouble, it is probably best to ask for help in the ISFL Dev & Wiki Discord.
What if I can't access Googledocs
The old guide has been superseded and is now located here. It still works, if you can't access Googledocs.
However, it does mean that there will be a lot more editing required.
If you use this method to make your page, you will need to tag 37thchamber or a member of the wiki team on discord for a clean-up before you can claim TPE.
